    Validity of Simpson-Angus Scale (SAS) in a naturalistic schizophrenia population

    BACKGROUND: Simpson-Angus Scale (SAS) is an established instrument for neuroleptic-induced parkinsonism (NIP), but its statistical properties have been studied insufficiently. Some shortcomings concerning its content have been suggested as well. According to a recent report, the widely used SAS mean score cut-off value 0.3 of for NIP detection may be too low. Our aim was to evaluate SAS against DSM-IV diagnostic criteria for NIP and objective motor assessment (actometry). METHODS: Ninety-nine chronic institutionalised schizophrenia patients were evaluated during the same interview by standardised actometric recording and SAS. The diagnosis of NIP was based on DSM-IV criteria. Internal consistency measured by Cronbach's α, convergence to actometry and the capacity for NIP case detection were assessed. RESULTS: Cronbach's α for the scale was 0.79. SAS discriminated between DSM-IV NIP and non-NIP patients. The actometric findings did not correlate with SAS. ROC-analysis yielded a good case detection power for SAS mean score. The optimal threshold value of SAS mean score was between 0.65 and 0.95, i.e. clearly higher than previously suggested threshold value. CONCLUSION: We conclude that SAS seems a reliable and valid instrument. The previously commonly used cut-off mean score of 0.3 has been too low resulting in low specificity, and we suggest a new cut-off value of 0.65, whereby specificity could be doubled without loosing sensitivity

    Looking for pyromania: Characteristics of a consecutive sample of Finnish male criminals with histories of recidivist fire-setting between 1973 and 1993

    BACKGROUND: As pyromania is a rare diagnosis with questionable validity, we aimed to describe a forensic psychiatric population of arson recidivists. METHODS: The medical records as well as the forensic psychiatric examination statements of 90 arson recidivists referred for pretrial psychiatric assessment in Helsinki University Hospital Department of Forensic Psychiatry between 1973 and 1993 were reviewed. RESULTS: The most important diagnostic categories of arson recidivists were personality disorders, psychosis and mental retardation, often with comorbid alcoholism. In all, 68% of arsonists were under alcohol intoxication during the index crime. Psychotic as well as mentally retarded persons with repeated fire-setting behaviour were mostly "pure arsonists"- persons guilty only of arsons during their criminal careers. Arson recidivists with personality disorder, in contrast, often exhibited various types of criminal behaviour and arson appeared to be only one expression of a wide range of criminal activity. Comorbid alcoholism was apparently a more rarely observed phenomenon among pure arsonists than in "nonpure arsonists". We found only three subjects fulfilling the present diagnostic criteria for pyromania. CONCLUSION: Using the criteria of the DSM-IV-TR, pyromania must be regarded as an extremely rare phenomenon. Especially the question of substance intoxication as an exclusion criterion for pyromania should be reconsidered

    Left prefrontal repetitive transcranial magnetic stimulation in schizophrenia

    In a double-blind, controlled study, we examined the therapeutic effects of high-frequency left prefrontal repetitive transcranial magnetic stimulation (rTMS) on schizophrenia symptoms. A total of 22 chronic hospitalized schizophrenia patients were randomly assigned to 2 weeks (10 sessions) of real or sham rTMS. rTMS was given with the following parameters: 20 trains of 5-second 10-Hz stimulation at 100 percent motor threshold, 30 seconds apart. Effects on positive and negative symptoms, self-reported symptoms, rough neuropsychological functioning, and hormones were assessed. Although there was a significant improvement in both groups in most of the symptom measures, no real differences were found between the groups. A decrease of more than 20 percent in the total PANSS score was found in 7 control subjects but only 1 subject from the real rTMS group. There was no change in hormone levels or neuropsychological functioning, measured by the MMSE, in either group. Left prefrontal rTMS (with the used parameters) seems to produce a significant nonspecific effect of the treatment procedure but no therapeutic effect in the most chronic and severely ill schizophrenia patients.Peer reviewe

    Aglomerasi Wilayah Kedungsepur terdiri dari 6 daerah yaitu Kota Semarang, Kota Salatiga, Kabupaten Semarang, Kabupaten Demak, Kabupaten Semarang, Kabupaten Kendal,  dan Kabupaten Grobogan. Daerah aglomerasi wilayah Kedungsepur memiliki karakteristik keunggulan berdasarkan sektor aktivitas ekonomi yang terjadi di daerah tersebut. Sektor unggulan sektor ekonomi suatu wilayah mengindikasikan tren perkembangan sektor ekonomi dan progresifitas perkembangannya jika dibandingkan dengan wilayah lainnya. Sektor unggulan ekonomi daerah tersebut akan terakumulasi menjadi kapasitas daerah dalam pengembangan internal daerah dan untuk sistem perwilayahan sekitarnya. Optimalisasi keunggulan sektor ekonomi tersebut dapat diperkuat dengan penguatan daya saing daerah secara internal, sehingga akan menciptakan nilai tambah sektor ekonomi disuatu wilayah. Daya Sang Wilayah tersebut terdiri dari Faktor Pendorong seperti Infrastruktur dan Kebijakan, Kapasitas Sumberdaya Manusia dan Tenaga Kerja, serta Kesiapan Inovasi dan Tenologi. Tujuan penelitian ini adalah untuk menganalisis peran daya saing dalam optimalisasi sektor unggulan daerah di Aglomerasi Wilayah Kedungsepur. Penelitian ini menggunakan analisis kuantitatif desktiptif dengan metode  Location Quotient dan Shift Share untuk menentukan sektor unggulan sektor ekonomi. Peran indeks daya saing dan inovasi daerah terhadap optimalisasi kapasitas wilayah tersebut dianalisis secara kuantitatif deskriptif. Hasil penelitian menunjukan masing-masing wilayah memiliki keunggulan sektor ekonomi yang berbeda dan saling terkait. Untuk mengoptimalkan produk unggulan tersebut perlu adanya penguatan masing-masing wilayah khususnya dalam kapasitas inovasi dan teknologi, pengembangan Sumber Daya Manusia dan Faktor Pendorong lainnya seperti infrastruktur, dan kebijaka

    Photoelectrochemical Activity Of Magnetron Sputtered ZnO Thin Films: Role Of Thermal Annealing

    This paper reports the effects of annealing temperature (varied from 250-450 ˚C) on the photoelectrochemical (PEC) efficiency of some highly dense zinc oxide thin films (ZOTFs) deposited on the indium tin oxide (ITO) substrate using RF magnetron sputtering method. The obtained TFs were characterized to determine their structures, morphologies, optical and PEC characteristics. The TF deposited at the photocurrent density of 0.034 mA cm-2 and photoconversion efficiency (0.026%) annealed at 400 °C for 1 hour was the optimum. Results from FESEM showed that the surface of the ZOTFs nanoparticles was very compact, with 17.75 nm at 400 ° C being the lowest particle size

    Oro-facial pain experience among symphony orchestra musicians in Finland is associated with reported stress, sleep bruxism and disrupted sleep-Independent of the instrument group

    Background To evaluate whether oro-facial pain experience was related to the type of musical instrument and to learn more about the roles of sleep and sleep-related issues in the pain among professional musicians. Objectives A standard questionnaire was sent to all Finnish symphony orchestras (n = 19), with altogether 1005 professional musicians and other personnel. Methods The questionnaire covered descriptive data, instrument group, items on perceived quality of sleep, possible sleep bruxism, stress experience and oro-facial pain experience during the past 30 days. Results In the present study, which included the musicians only, the response rate was 58.7% (n = 488). All orchestras participated in the study, and there was no significant difference in the response rate between the orchestras. The mean age of men (52.3%) was 47.7 (SD 10.3) and of women (47.7%) was 43.4 (SD 9.8) years (P <0.001). Overall, current pain in the oro-facial area was reported by 28.9%, frequent bruxism by 12.1% and frequent stress by 20.8%. According to Somers' d, there were statistically significant but moderate correlations between overall pain reports in the oro-facial area and disrupted sleep (d = 0.127, P = 0.001), sleep bruxism (d = 0.241, P <0.001) and stress experiences (d = 0.193, P <0.001). Logistic regression revealed, independent of the instrument group (string, woodwind, brass wind, percussion), that current oro-facial pain experience was significantly associated with disrupted sleep (P = 0.001), frequent sleep bruxism (P <0.001) and frequent stress (P = 0.002) experiences. Conclusions Among symphony orchestra musicians, oro-facial pain experience seems to be related to perceptions of stress, sleep bruxism and disrupted sleep rather than the instrument group.Peer reviewe

    Association between psychological distress and liver disease mortality: a meta-analysis of individual study participants

    Background & Aims: Risk factors for cardiovascular disease, such as obesity and hypertension, have been associated with non-alcoholic fatty liver disease. Psychological distress (symptoms of anxiety and depression) is a risk factor for cardiovascular disease, so it might also be associated, directly or indirectly, with elevated rates of liver disease. We investigated the relation of psychological distress (measured by the 12-item General Health Questionnaire; GHQ) with liver disease mortality. Methods: We performed a meta-analysis of data from individual participants in 16 prospective studies of the general population of the UK, initiated from 1994 through 2008. We categorized GHQ score into four groups: zero (no distress), 1-3, 4-6, and 7-12. Results: We used data from 166,631 individuals (55% women; age, 46.6±18.4 years; range, 16−102 years). During a mean follow-up period of 9.5 years, 17,368 participants died (457 with liver disease). We found a significant increase in risk for liver disease mortality as GHQ score increased across categories (ptrend<0.001). The age- and sex-adjusted hazard ratio for the highest GHQ category (7-12) compared to those scoring zero was 3.48 (95% confidence interval, 2.68−4.52). After adjustment for health behaviors, socioeconomic status, body mass index, and diabetes, the hazard ratio was partially attenuated to 2.59 (95% confidence interval, 1.82–3.68). Conclusions: Our novel finding that psychological distress was associated with liver disease mortality requires testing in other studies. Though results are unlikely to be causal, we provide further evidence for the deleterious effects of psychological problems on physical health

    Detecting suicidality among adolescent outpatients: evaluation of trained clinicians' suicidality assessment against a structured diagnostic assessment made by trained raters

    <p>Abstract</p> <p>Background</p> <p>Accurate assessment of suicidality is of major importance. We aimed to evaluate trained clinicians' ability to assess suicidality against a structured assessment made by trained raters.</p> <p>Method</p> <p>Treating clinicians classified 218 adolescent psychiatric outpatients suffering from a depressive mood disorder into three classes: 1-no suicidal ideation, 2-suicidal ideation, no suicidal acts, 3-suicidal or self-harming acts. This classification was compared with a classification with identical content derived from the Kiddie Schedule for Affective Disorders and Schizophrenia (K-SADS-PL) made by trained raters. The convergence was assessed by kappa- and weighted kappa tests.</p> <p>Results</p> <p>The clinicians' classification to class 1 (no suicidal ideation) was 85%, class 2 (suicidal ideation) 50%, and class 3 (suicidal acts) 10% concurrent with the K-SADS evaluation (γ<sup>2 </sup>= 37.1, df 4, p = 0.000). Weighted kappa for the agreement of the measures was 0.335 (CI = 0.198–0.471, p < 0.0001). The clinicians under-detected suicidal and self-harm acts, but over-detected suicidal ideation.</p> <p>Conclusion</p> <p>There was only a modest agreement between the trained clinicians' suicidality evaluation and the K-SADS evaluation, especially concerning suicidal or self-harming acts. We suggest a wider use of structured scales in clinical and research settings to improve reliable detection of adolescents with suicidality.</p

    A structural variation reference for medical and population genetics

    Structural variants (SVs) rearrange large segments of DNA(1) and can have profound consequences in evolution and human disease(2,3). As national biobanks, disease-association studies, and clinical genetic testing have grown increasingly reliant on genome sequencing, population references such as the Genome Aggregation Database (gnomAD)(4) have become integral in the interpretation of single-nucleotide variants (SNVs)(5). However, there are no reference maps of SVs from high-coverage genome sequencing comparable to those for SNVs. Here we present a reference of sequence-resolved SVs constructed from 14,891 genomes across diverse global populations (54% non-European) in gnomAD. We discovered a rich and complex landscape of 433,371 SVs, from which we estimate that SVs are responsible for 25-29% of all rare protein-truncating events per genome. We found strong correlations between natural selection against damaging SNVs and rare SVs that disrupt or duplicate protein-coding sequence, which suggests that genes that are highly intolerant to loss-of-function are also sensitive to increased dosage(6). We also uncovered modest selection against noncoding SVs in cis-regulatory elements, although selection against protein-truncating SVs was stronger than all noncoding effects. Finally, we identified very large (over one megabase), rare SVs in 3.9% of samples, and estimate that 0.13% of individuals may carry an SV that meets the existing criteria for clinically important incidental findings(7). This SV resource is freely distributed via the gnomAD browser(8) and will have broad utility in population genetics, disease-association studies, and diagnostic screening.Peer reviewe

    The mutational constraint spectrum quantified from variation in 141,456 humans

    Genetic variants that inactivate protein-coding genes are a powerful source of information about the phenotypic consequences of gene disruption: genes that are crucial for the function of an organism will be depleted of such variants in natural populations, whereas non-essential genes will tolerate their accumulation. However, predicted loss-of-function variants are enriched for annotation errors, and tend to be found at extremely low frequencies, so their analysis requires careful variant annotation and very large sample sizes(1). Here we describe the aggregation of 125,748 exomes and 15,708 genomes from human sequencing studies into the Genome Aggregation Database (gnomAD). We identify 443,769 high-confidence predicted loss-of-function variants in this cohort after filtering for artefacts caused by sequencing and annotation errors. Using an improved model of human mutation rates, we classify human protein-coding genes along a spectrum that represents tolerance to inactivation, validate this classification using data from model organisms and engineered human cells, and show that it can be used to improve the power of gene discovery for both common and rare diseases.Peer reviewe
